I The problem

When a dispute, an audit or a regulator arrives, most organisations cannot show who decided, on what information, under what authority, or when the decision was due for review.

II Approach

  1. Record every significant decision on the Decision Canvas: twelve fields, in four lighter variants for smaller decisions.
  2. Review decision governance against the Decision Governance Standard: seventeen principles in four pillars.
  3. Score each pillar and produce a remediation plan a board can track.
  4. Keep people deciding: the canvas records the decision; it never makes it.
